Notice of Privacy Practices

Exetazo is required, when operating as a covered entity or through covered clinicians, to maintain the privacy of protected health information (PHI), give you this notice, and follow the notice that is in effect.

Uses and disclosures without extra authorization

Treatment. We may share PHI with your Exetazo clinician, covering clinicians, pharmacies, laboratories, and, when needed, emergency personnel.

Payment. We may use PHI to bill you, process refunds, or (if we later accept insurance) submit claims. Today Exetazo is cash-pay unless we tell you otherwise.

Health-care operations. We may use PHI for quality review, credentialing, training, customer support, and fraud prevention.

As required by law. Public health, abuse reporting, health oversight, judicial process, law enforcement, coroners, organ donation, research with approval, serious threats to health or safety, and workers’ compensation, as the law allows.

Uses that need your written authorization

Most uses of psychotherapy notes (if any), most marketing, and sale of PHI require your authorization. You may revoke an authorization in writing, except to the extent we have already relied on it.

Your rights

You may ask to inspect or get a copy of your record; ask us to amend it; request confidential communications; request extra limits; get a list of certain disclosures; get a paper copy of this notice; and choose someone to act for you. You may file a complaint with Exetazo via the Contact page or with the U.S. Department of Health and Human Services. We will not retaliate for a complaint.

Our duties

We must notify you of a breach of unsecured PHI as required by law. We may change this notice. The new notice will be posted on this site and will apply to information we already have.
