Telehealth informed consent

You must agree to this consent before a visit is booked. If you think you have an emergency, call 911. Do not use telehealth for emergencies. Exetazo is not an on-call service.

By booking or attending an Exetazo telehealth visit, you (or the parent/guardian of a minor) consent to receive care by electronic means (audio, video, or secure messaging as offered).

What telehealth is

A clinician licensed in the state where you are physically located evaluates you remotely. The standard of care is the same as an in-person visit of similar type, but the clinician cannot examine you in person and may decide you need a clinic, urgent care, or emergency department instead.

Benefits and limits

Benefits include faster access and lower travel cost. Limits include technology failure, incomplete visual exam, delayed labs or imaging, and privacy risks if you take the visit in a public place or on an unsecured network. You agree to take the visit in a reasonably private setting.

Scheduled care, not on-call

Exetazo does not operate an on-call or after-hours coverage line. You book a visit at a time a licensed clinician in your state has posted, and that is when you are seen. If no clinician has posted a time, that hour is not covered. Messages sent when no slot is open are not an emergency or on-call consult.

Your responsibilities

Privacy

Health information is handled as described in the Notice of Privacy Practices and Privacy Policy. Electronic visits can be interrupted. Do not record the visit unless the clinician agrees and the law allows it.

Prescriptions

A clinician may prescribe when appropriate and lawful. Controlled substances and some other drugs have extra federal and state rules. A prescription is never guaranteed.

Right to refuse

You may refuse telehealth and seek in-person care instead. Refusing telehealth will not affect your right to future lawful care, but Exetazo may not offer an in-person alternative.

Acknowledgment

You acknowledge that you have read this consent, the Notice of Privacy Practices, and the Terms of Use, that you understand them, and that you have had a chance to ask questions via Contact or Support before booking.
